Background: This systematic review investigated the effectiveness, efficiency and apical extrusion of the debris of two rotary and two reciprocating single-file systems used for the removal of filling material from straight root canals. Material and Methods: A literature search was performed in the Medline, ISI Web of Science, and Scopus databases for relevant articles matching the keyword search strategy. Effectiveness was determined with studies dealing with the ability of the instruments to remove filling material from root canals. Efficiency was assessed with studies dealing with the time needed to completely remove the root canal filling, and apical extrusion was determined with studies that measured the amount of filling material extruded through the apex. Results: From the 424 articles initially found, 406 were excluded for being non-relevant or not fulfilling the selection criteria. Another 9 articles were excluded after methodology evaluation. Finally, 9 studies were included in the systematic review. Conclusions: None of the reviewed systems is effective to completely remove the filling materials from straight root canals, and all systems appear to be equally time-efficient, although this variable shows different results. In terms of apical extrusion, the analyzed reciprocating systems extrude more material toward the periapical tissues than the continuous rotation systems. El objetivo fue evaluar la eficacia de remoción del material de obturación y el tiempo empleado para la desobturación con tres métodos diferentes, en modelos réplica. Se utilizaron 24 modelos réplica de premolares inferiores instrumentados con sistema Protaper Gold hasta F4, irrigación NaOCl 2,5% y ED-TAC 17%. Obturación termoplastizada sistema Fast Pack Pro. La muestra (n=24) se dividió aleatoriamen-te en tres grupos experimentales (n=8) sometidos a distintos métodos de desobturación. Grupo 1: fresas Gates Glidden II/III y limas Hedstroem. Grupo 2: lima Medium sistema Wave One Gold y punta ultrasóni-ca Ultra X, (Eighteeth). Grupo 3: lima Rotate 35/04 y punta ultrasónica R1 Clearsonic, (Helse). Se midió el tiempo de desobturación. Las piezas se radiogra-fiaron con radiovisiógrafo digital RVG 5200 (Cares-tream), y fueron procesadas con software Image-J. Al analizar cantidad de material de obturación rema-nente, la prueba de Kruskal-Wallis (p<0,05), mostró diferencias estadísticamente significativas entre grupos 2 y 3. Grupo 1 no mostró diferencias signifi-cativas con los otros dos (p>0,05). Al analizar tiempo de desobturación, el test de Kruskal-Wallis no deter-minó diferencias significativas entre grupos 1 y 2 (p>0,05), el grupo 3 tuvo diferencias estadísticamen-te significativas con los grupos 1 y 2 (p<0,05). En con-clusión, ninguno de los sistemas de desobturación evaluados logró eliminar la totalidad del material de obturación. El que combinó limas rotatorias con punta ultrasónica de retratamiento fue el que mos-tró mayor efectividad de remoción y demandó menor tiempo de trabajo (AU)


Objective: to evaluate the effectiveness of obturation material removal and the time that the procedure took, when performing the retreatment with three different methods, in replica models of mandibular premolars. Materials and methods: 24 replica models of lower premolars instrumented with Protaper Gold system up to F4, 2.5% NaOCl irrigation and 17% ED-TAC were used. Thermoplastized sealing with Fast Pack Pro system. The sample (n=24) was randomly divided in three experimental groups (n=8) subjected to different unsealing methods. Group 1: Gates Glid-den burs II and III and Hedstroem files. Group 2: Wave One Gold Medium file system and Ultra X ultra-sonic tip, (Eighteeth). Group 3: Rotate 35/04 file and R1 Clearsonic ultrasonic tip (Helse). Unsealing time was measured. The samples were radiographed with a digital radiovisiograph RVG 5200 (Carestream), and processed with Image-J software. When analyzing the amount of remaining filling material, Kruskal-Wallis test showed statistically significant differences be-tween groups 2 and 3 (p<0,05). Group 1 did not show significant differences with the other two (p>0,05). When analyzing unsealing time, Kruskal-Wallis test determined that there were no significant differ-ences between groups 1 and 2 (p>0,05), but group 3 had statistically significant differences with the other two (p<0.05). None of the unsealing systems evalu-ated managed to eliminate all of the sealing material. The group that combined rotary files with ultrasonic retreatment tip showed the greatest removal effec-tiveness and required less work time (AU)

BACKGROUND: Current treatment of immature necrotic permanent teeth with a periapical lesion is regenerative endodontics, which is based on tissue engineering under the triade of stem cells, scaffolds and bioactive molecules. OBJECTIVES: This Umbrella Review was aimed to evaluate the success of scaffold and regenerative materials used for the treatment of these teeth, in terms of apical closure, tooth length increase, widening of root canal walls, tissue vitality and periapical lesion repair. METHODS: An extensive literature research was carried out in the Medline, ISI Web of Science, and Scopus databases for relevant systematic reviews matching the keyword search strategy. Based on inclusion and exclusion criteria, reviewers independently rated the quality of each study to determine their level of evidence. Methodological quality assessment of each article was obtained using A Measurement Tool to Assess Systematic Reviews (AMSTAR)-2 tool, and risk of bias was assessed with the Risk of Bias in Systematic Reviews (ROBIS) tool. RESULTS: After removing duplicates, 155 articles were found; from which 133 were excluded for being non-relevant and 15 other due to exclusion criteria. One more was discarded after methodological quality evaluation, for a total of six articles remaining. The most common scaffold used was the blood clot, others used were poly lactic-co-glycolic acid and platelet-rich fibrin matrix. The most common regeneration material used was Mineral Trioxide Aggregate (MTA), followed by Biodentine. An increase in tooth length and widening of root canal walls were reported in all selected studies with different proportions, as well as periapical lesion repair. ROBIS analysis showed that only one article had low bias, two were classified as unclear bias, while the remaining three had high risk of bias. DISCUSSION: An exhaustive literature search was carried out applying language filters, high-quality indexed journals, year of publication, which ensures the best quality articles were included. Blood clot was the most used scaffold as is the most easy to place inside the canal and does not require to extract blood from the patient. The use of MTA and Biodentine as sealing materials has been associated with thickening of canal walls, apical closure and reduced signs and symptoms of apical periodontitis. However, most of the included reviews assessed were case reports and only in a few of them were clinical trials included. There is also a lack of risk of bias analysis in most reviews. CONCLUSION: The blood clot is the most common scaffold used for inducing regeneration during the treatment of immature necrotic teeth. Tooth length increase and widening of root canal walls are the most common criteria used in the studies as success indicators. MTA and Biodentine did not show differences in the results analysed. Quality assessment and bias risk evaluation showed that it is necessary to design better studies with rigorous methodology to recommend a trustable and predictable protocol for the treatment of immature necrotic permanent teeth with periapical lesions. PURPOSE: To compare fiber posts of several calibers and trademarks to their corresponding root canal preparation drills. METHODS: Three widely used endodontic post brands and their drills were evaluated: Exacto, ParaPost Taper Lux, and Macro-Lock Illusion X-RO. Fiber posts and drills were microphotographed with a scanning electron microscope and images were analyzed using ImageJ image processing software. Fiber post diameter on apical extreme (Pd0), fiber post diameter at 5 mm from the apical extreme (Pd5), drill diameter on apical extreme (Dd0) and drill diameter at 5 mm from the apical extreme (Dd5) were analyzed. The data were statistically analyzed using student t-test. RESULTS: Exacto posts 0.5 showed larger dimensions than their corresponding drills (P< 0.05) at Pd0. Macro-Lock posts showed no significant differences vs. their drills at Pd0 in any of the studied groups. ParaPost drills 4.5, 5 and 5.5 were statistically significantly larger than their posts at Dd0 (P< 0.05). Exacto posts 0.5 and 1 showed larger dimensions than their drills measured at Pd5 (P< 0.05). Exacto posts number 2 showed smaller calibers than their corresponding drills at Pd5 (P< 0.05). Macro-Lock drills number 4 and ParaPost drills number 5 were larger than their posts at Dd5 (P< 0.05). CLINICAL SIGNIFICANCE: Poor spatial correspondence between post and drill dimensions can adversely affect the film thickness of the resin cement, diminishing bond strength due to polymerization shrinkage. The lack of correspondence in size between posts and drills may lead to the formation of empty chambers between the post and endodontic obturation with excessive luting cement thickness, thus inducing critical C-Factor stresses.

Introducción: el diagnóstico en endodoncia incorporó los estudios de tomografías de haz cónico (CBCT). Esnecesario entender el alcance y la utilidad de estos para saber cuándo solicitarlos. Objetivos: el objetivo de este estudio fue determinar la indicación de CBCT previa al tratamiento endodóntico y su relación con el nivel de dificultad del caso atratar. Métodos: se seleccionaron al azar 15 casos clínicos de alta dificultad derivados a tratamiento de endodoncia.Se realizó diagnóstico clínico y radiográfico. Se estimaron los recursos necesarios para el tratamiento con unformulario diseñado para ello. Se solicitó un estudio de CBCT. Una vez obtenido el estudio, el mismoprofesional que realizó el diagnóstico clínico navegó el software de visualización y llenó nuevamente el formulario. Fueron comparados los formularios previos y posteriores a las CBCT. Se establecieron las diferencias encontradas entre ellos. Resultados: todos los casos analizados tuvieron diferencias en la comparación entre los formularios. Conclusiones: el uso de CBCT en los casos de elevada dificultad permite un diseño de plan de tratamiento más preciso.


Introduction: the endodontic diagnosis includes the study of cone beam tomography (CBCT). It is necessary to understand the scope and usefulness of CBCTto know when to request it.Objectives: the aim of this study was to determine the requirement of CBCT before endodontic treatment and its connection with the level of difficulty that the case presents. Methods: 15 endodontic cases of high difficulty were taken randomly. Clinical and radiographic diagnosis was made. The necessary resourcesfor the treatment were estimated according to aspecially designed form. A CBCT study was request. Once the study was obtain, the same professionalwho performed the clinical diagnosis browsed the software to read CBCT results and filled a new form.Both CBCT forms need to be compared to establish differences between them. Results: all the cases analysed were compare, andshowed differences between the two forms. Conclusion: the use of CBCT in high difficult cases provides a more accurate design of the treatment plans.

La práctica endodóntica requiere el respaldo de imágenes de manera permanente, tanto para el diagnóstico como durante las distintas etapas del tratamiento. Desde siempre, estas imágenes han sido radiográficas. Con el advenimiento de las tomografías, se incluyó la visión de la tercera dimensión y se logró despejar interferencias gracias a la incorporación de cortes sectorizados. Sin embargo, estos estudios implican el uso de radiaciones ionizantes y de aparatología de alto costo. Con el tiempo, los estudios tomográficos han evolucionado; permiten obtener imágenes más fieles, isométricas, con mejor definición ymenos dosis de radiación para los pacientes. El presente trabajo tiene como objetivo brindar información clara a los colegas que deseen incorporar estas nuevas tecnologías en su práctica cotidiana. Para ello, se describen las diferencias entre los distintos sistemas, la manera en que deben ser evaluados por el profesional y los límites de su aplicación...
